Required Release Form
In order to participate in the clinic we need a WAIVER FORM with both sides completed on file. Please print, complete, then either scan or take a picture of both sides and email as an attachment to crimsonsoccerclinics@gmail.com.
The WAIVER FORM must be signed by a parent or guardian if participant is under 18 years old. Participants who are 18 years old or will become 18 years old before the end of the camp/clinic must also sign. Thanks!
